Financial Policy 1. Once treatment begins, no refund will be issued on any Special Package Pricing. 2 .Deposits of $100 are required on treatments costing over $300 and are non-refundable. 3. All treatments are non-transferable to other services and non-assignable to any other person other than the patient. 4. Any remaining LipoDissolve treatments after completion of therapy can be used by the patient on any other cosmetic zone as approved by the provider. Appointment Cancellation Policy This office requires a 24 hour cancellation notice for all services. You will be charged a $50 fee prior to your next appointment for any appointment cancelled less than 24 hours or "no show" appointment. Your credit card on file will be billed the day of the cancellation or "no show". Children Childcare services are not provided. Out of respect for the privacy of our guests and the safety of your child, we ask that children not accompany you to a scheduled appointment. Minors are welcome for appropriate treatments if accompanied by a parent or will a prior signed waiver. |
